Before Coming to Class:
Before you come to your first class, please print and fill out this Waiver of Liability and Medical Release. Please and thank you!
Payments:
All payments are due by the 5th of every month. After the 10th, your child will not be able to participate in class until their account is paid in full.
Shoes:
Each class requires specific shoes. This is not optional and must be purchased prior to starting lessons. Please contact me with any questions you have regarding shoes.
